A quick guide to viewing and acting on key check-ins from the Action Plan dashboard

🩺 Why this matters


Check-ins are a simple way to track a Person’s well-being—before, during, or between visits. The Check-in Results block highlights the latest results from a specific check-in, directly on the dashboard. This ensures the most important updates are always easy to find.

Use it to:

  • Monitor health trends over time

  • Prioritize check-ins that are key to your care pathway

  • Support proactive, informed decisions

Whether you’re tracking pain, mood, or a PPS score, the results stay front and center.

🔍 Where to find it

  • Go to the People tab

  • Select the patient and the relevant Action Plan

  • Find the Check-in Results block on the Action Plan dashboard

You’ll see:

  • The latest result

  • A trend arrow indicating change

  • The name of the person who completed it

  • A link to view full history

🚀 What you can do in this section

 

📅 View trends and history

  • See if results are improving, worsening, or staying stable

  • Click to view past answers and who completed them

  • Supports handovers and shared decision-making

🖊️ Complete a check-in

  • Click the +Add button in the Check-in block or +Add in the Things to do block

  • In the pop-up modal, answer the Check-in questions.

  • Click to view results and see past history of Check-in answers

  • Can be filled out by the Person, their Support Team, or the Health Team

🔐 Set who can view the check-in

  • Some check-ins are only visible to the Health Team

  • Others are shared with the Person and their Support Team

💡 Tips and things to know

  • Check-in formats include: yes/no, scales (1–5, 0–10), mood, numeric, multiple choice, free text, likhart scales

  • You can use Check-ins for internal team tracking or collaborative updates

  • Health Team-only check-ins are a great way to track clinical insights like PPS without overwhelming families

  • Check-ins differ from Forms: they’re faster to complete and focus on a small number of questions

The Check-in Results block helps your team stay focused, responsive, and person-centered—right from the dashboard.
