š What it is
Careteam makes cross-organization collaboration simple, secure, and person-centred. When multiple health teams are involved in a Personās care, Careteam acts as the shared workspaceāconnecting everyone around a single Action Plan.
Think of it as the interstitial glue between programs, clinics, and servicesābringing everyone (including the Person and their supporters) onto the same page in real time.
š How it works
When one organization starts an Action Plan, they can invite other organizations in their network to join that plan with consent from the Person.
Once invited:
The receiving organization is notified and can accept or decline
Upon acceptance, the Person appears in their People list
The plan is labeled as "Shared Action Plan from [originating organization]"
The dashboard looks the same for all health teams, but each team can only edit their own contentāensuring clarity and accountability.
š ļø What each team can do
Invited health teams can contribute to everything in the Action Plan dashboard. Everything is visible instantly to the Person (and their supporters) and to all participating health teamsācreating a truly dynamic, real-time care plan.
š How to invite another health team
Go to the Personās Team section in their Action Plan
Click + Add Organization
Select a Organization from the network
Confirm that you have consent
Click Invite
Once accepted, that health team joins the shared workspace and can begin contributing.
