A quick guide to making and tracking referrals directly within your Action Plan

🩺 Why this matters


Referrals are a critical part of patient care transitions. The Services and Referrals block gives you a centralized place to:

  • Make electronic referrals through CareDove

  • Track referral status within Careteam

  • Share visibility across teams and organizations

  • Reduce duplication and avoid missed steps

This streamlines coordination, reduces duplication, and helps ensure nothing falls through the cracks.

🔍 Where to find it

 

  1. Go to the People tab

  2. Select the patient and relevant Action Plan

  3. Look for the Services and Referrals block on your Action Plan dashboard. 

  4. Click + Add to launch the referral process

 

🚀What you can do in this section


🔗Make electronic referrals

Use the Add button to launch CareDove directly from Careteam.

  • If the patient’s home address is in Careteam, it will auto-populate

  • Check the box to confirm consent to share patient information (name, DOB, address, phone)

  • If you have a CareDove account, you'll log in via Single Sign-On (SSO)—this links your Careteam activity to your CareDove account

  • If you don’t, you’ll be using a Careteam shared account. You can still make referrals, and the receiving organization will have your contact info for follow-up

You can book services like Meals on Wheels, select dates/times, and submit the referral seamlessly.

📊 Track referral status


Referral updates made in CareDove sync back instantly with Careteam. Everyone stays informed.

Once submitted, the referral appears in the Action Plan with a status such as:

  • Sent, Pending, In-progress

  • Booked, Completed, Rejected, Cancelled, or On-hold

🤝 Ensure visibility across care teams

Referrals are visible to:

  • All internal team members

  • The patient or family (if the plan is shared)

  • External organizations via Shared Action Plans

This helps avoid duplicated referrals and ensures everyone is working from the same information.

  • A Service Library, much like your existing Resource Library

  • The ability to recommend non-integrated services

  • Tracking for manual referrals

💡 Tips and things to know

  • All referrals made through Careteam are logged in your CareDove account

  • This reduces the risk of duplicate work across teams

  • External collaborators can see referrals when invited into a Shared Action Plan

The Services and Referrals block helps streamline transitions, close loops, and ensure care teams are aligned across settings.
