A quick guide to adding family, friends, or trusted contacts to support someone in Careteam

🩺 Why this matters
Support team members—like family or friends—play a vital role in person-centred, integrated care. By inviting them to the Action Plan, you help ensure everyone stays connected, informed, and able to contribute meaningfully to the care journey.
In Careteam, you can:
Invite someone to join the Action Plan directly
Or simply add their contact info for future reference
This makes it easy to personalize the care circle based on what the Person wants and needs.
🔍 Where to find it
Go to the People tab
Select the Person’s name
Choose the correct Action Plan
Click the Team tab (e.g., "Holly’s Team")
🚀 What you can do in this section
👥 Add a support team member
Click + Add to Team
Choose Support Team Member
Fill in their name, relationship, and email
Choose whether to:
Invite them to collaborate on the Action Plan
Or just add their contact info (optional)
🧑🤝🧑 Choose their access level
Role Access Level Description Primary Supporter Most access Trusted contact acting on behalf of the Person (e.g., digital representative). Can complete tasks, send messages, and upload documents. Supporter View + limited interaction Family/friend with view access who can support with tasks and messages.
📨 Send the invitation
Check that the email is correct
Confirm you have consent from the Person to share their health information
Click Send Invitation
Once sent:
You’ll see the contact listed with a Pending status
This updates to “Accepted” once they join
You can resend or invite later if they were added as a contact only
💡 Tips and things to know
You can invite health team members in a similar way—great for involving community providers not already on Careteam
Pending invites stay visible so you know who’s joined and who hasn’t
The support team can be updated anytime based on the Person’s wishes
The Support Team tab helps you build a circle of care that reflects what truly matters to the Person—and keeps everyone in sync.
