The Resource block in the Action Plan is where all the essential resources for the person and their support team are centrally located. These resources can include documents (PDF, DOC/DOCX, PPT/PPTX, XLS/XLSX, CSV, JPEG, GIF, and PNG files), electronic links to websites or videos, and even contact information for other providers or services.

You can add resources to a Person's Action Plan either through the Resource Library or by uploading one-time resources directly from your computer. Content in the Resource Library consists of resources often shared with the population you serve, such as fact sheets, websites, or videos that are helpful to most People. These resources are managed by your organization’s administrator (see the article on managing content in the Resource Library). If you need to add a resource to the library, reach out to your organization's administrator or contact us.

In the meantime, you can always add one-time resources to individual Action Plans. These one-time resources are typically specific to that Person, such as test results, personalized education, or a discharge note.

To add Resources to an Action Plan:

  1. Click the +Add button in the Resource block.
  2. In the pop-up modal, select a Resource from your organization’s Library or upload a one-time resource directly from your computer.

Cool things to know:

  • By centralizing resources, everyone involved in the Person’s care stays informed and connected, no matter where they are. Additionally, if you are collaborating with other Health Teams, you can see the Resources they’ve added, both preventing duplication and ensuring that nothing important is missed. This inspires seamless collaboration and shared responsibility across the entire care team around the Person.
  • The Person and their Primary Supporters (their main caregivers who are enabled to act as them) can also add Resources, which then acts as their digital binder. This means they can use the Resource block as their own central storage for important documents or even share Resources they’ve received from healthcare providers who aren’t using Careteam. This ensures that all critical information is shared with the Person’s Health Team, keeping everyone informed and connected.
  • It may be a great way to share some of these beyond this Person account in order to crowdsource links and services that patients find useful, but Resources outside of the Library that are added into this block for this Person are only seen within this Person’s Action Plan. 

By using the Resource block effectively, you ensure that everyone involved in the person’s care is informed and connected, with access to the most important information and tools they need.
