Streamline your referral process and enhance patient care coordination with our integrated solution. In this article, we'll show you how to make referrals to other health and community organizations seamlessly, leveraging the power of our partnership with Caredove. Whether you're referring to a specialist, a community service, or any other organization, discover how this efficient system automatically populates patient information to prevent duplication and provides real-time visibility into the referral status. Elevate your referral process and patient care with ease.

Follow these steps to make a referral: 

  1. Click on the “People” tab to see the list. 
  2. Select the “Patient” name and Action Plan you want to make a referral for.
  3. Within the Action Plan tab scroll down to the Referrals section and click “+Add”.
  4. If the patient's home address is in Careteam, it will automatically populate in the Caredove pop-up. If not, you'll need to enter it directly within the Caredove pop-up.
  5. Check the box  (Launch Caredove and pass this person’s contact information (name, date of birth, address, phone). 
  6. If you have a Caredove account, check the box and click 'Next'.                                                                                                                                                                                                                                                                                                                                            Already have a Caredove account: You will be taken through the steps of Single Sign On (SSO) with Caredove. This will link your account and ensure that actions taken within Careteam are linked to your Caredove account.                                                                                                                                                                                                                                                                                                                                                                                                                                                              Do not have a Caredove account: You have full access to Caredove to make referrals. The account you are using is a general a Careteam account.  The receiving organization will have access to your contact details and can directly reach out via email or phone in case of specific inquiries.                                                                                                                                                                                                                      
  7. Complete your desired referral by following the steps within Caredove. 
  8. Once complete, the referral will be listed within the Action Plan as Sent, Pending, In-progress, Rejected, Cancelled, On-hold, Completed, or Booked.
