The Person’s Panel sits at the top of every Action Plan dashboard—on desktop and mobile—and never changes position. It’s your go-to place for key context: who's involved, what's important to the Person, and what information you need to personalize and coordinate care.

It’s one of Careteam’s foundational building blocks for delivering person-centered, proactive, and integrated care.


šŸ“‹ What you’ll find in the Person’s Panel

Click to open each section:

  • šŸ‘„ Team:

    • Lists everyone involved in the Person’s care, including:

      • Support Team (family, friends, primary supporters)

      • Health Team (care providers from one or more organizations)

    • Each name includes their role and contact method, so you always know who’s who.

  • šŸŽÆ Goals:

    • Displays goals added by the Health Team, the Person, or their Support Team

    • Encourages shared understanding of what matters most

    • You and others can add, edit, and mark goals as completed

  • šŸ“‘ Information:

    • Shows the Person’s contact details, demographics, and digital binder

    • Clients or their primary supporters can use this as their space to capture:

      • Health history

      • Key notes

      • Anything they want the care team to know

    • Health providers can view this content and edit core info like phone number, email, or PCP

  • šŸ—‚ļø About This Action Plan:

    • See or edit:

      • The Action Plan name (often aligned to the program, e.g. ā€œPalliative Pathwayā€)

      • Key instructions for the team

      • Concerns or guidance unique to this care journey

      • Archive or unarchive the Action Plan if appropriate

  • Encourage people to add their own health history or notes under the Information section—it gives you richer context

  • Keep Action Plan instructions up to date as care evolves

  • Use the Person’s Goals as a shared, motivating focal point across teams

  • Everything in the Person’s Panel stays visible no matter how the rest of the dashboard is configured


The Person’s Panel is a simple yet powerful tool to drive better coordination and stronger relationships. It’s not just about the data—it’s about understanding the Person at the center of care.
