The Action Plan is where you’ll find everything related to a Person’s Care Plan (read about why Careteam uses Person instead of Patient and Action Plans instead of care plans). The Action Plan dashboard is customizable, allowing you to tailor a template that meets both the unique needs of the People you serve and the goals of the Health Team.

 

Key capabilities:

  • About the Person: Consistently positioned in the top left corner of the computer screen of every Action Plan, this panel includes the Team, Goals, Personal information, and details About the Action Plan (on mobile this is the first block).. 
  • My Profile, Notifications, and Secure communication: These are located in the top right corner of the dashboard on a computer screen for easy access.
  • Modular design: The rest of the Action Plan template is customizable. You can choose what blocks go into an Action Plan and where they are placed as you set up the template. This allows you to create specific dashboard layouts tailored to the various populations you serve (e.g., Hospital to Home, Caregivers, Dementia, etc.). Whatever configuration you choose to create, this is what your patients and caregivers will see.
  • With great power comes great responsibility: Once created, the content of an Action Plan remains flexible and can be updated to keep everyone up to date as the Person progresses along the pathway.

Cool things to know:

  • Most Action Plans will include the Things to do and Resources blocks:
    • Things to do: Lists the key activities for the Person, their Support Team or the Health Team such as Appointments, Check-ins, Forms, Tasks, and Referrals that need to be completed to ensure the right action happens at the right time.
    • Resources: Provides key educational content, including documents and links to websites and videos available to the Person and their Support Team.
  • There are many other blocks, such as Check-In, Checklist, and General information.  
  • There are two options for where these blocks can be presented (note: regardless of their location, the blocks function the same way): 
    1. In the main panel, where they appear larger, or
    2. In the side panel under the About the Person section, where they appear smaller.
