-
Navigate to archived Action Plans: In the patient's list of Action Plans, locate and access the "Archived Action Plans" section.
-
Open the appropriate archived Action Plan: Identify and open the specific archived Action Plan that needs reactivation.
-
Reactivating the Action Plan: Within the Details section of the Action Plan, click on the "Re-activate" option.
-
Confirmation: A confirmation prompt will appear. Click "Okay" to confirm and initiate the reactivation process.
Effects of reactivation:
Upon reactivating the archived Action Plan:
- The patient will receive a new invitation to activate the Action Plan.
- This reactivation enables the patient to regain visibility and access to the previously archived plan.
This straightforward process ensures a seamless transition for patients back into the Careteam platform and re-establishes their access to their health-related Action Plans.
Reactivating archived Action Plans empowers healthcare providers to resume patient engagement, ensuring continuity and seamless access to critical health information.
