A quick guide to ending an Action Plan when care is complete—or no longer active

🩺 Why this matters
There are many reasons to archive an Action Plan: the care pathway may be complete, the client may have been discharged, or the client/family may request to no longer participate. Archiving helps keep dashboards clean and focused—without losing access to important health information.
Key points:
Archiving removes access for the Person and their Support Team
Health team members retain full access for reference, audits, or re-engagement
Plans can be unarchived at any time if needed
🔍 How to archive an Action Plan
Open the Person’s profile
From the People tab, select the client or patient
Choose the correct Action Plan
If the person has multiple Action Plans, be sure to open the one you want to archive
Go to “About this Action Plan”
Scroll down to find the Archive option
Click “Archive” and confirm
You’ll be prompted to confirm—once done, the Action Plan will move to the archived section
🔓 How to unarchive an Action Plan
From the same Person profile, scroll to Archived Action Plans
Select the plan you wish to reactivate
Click “Unarchive”—the plan will return to active status
📝 Tip: This is helpful if a client re-engages in care and you want to pick up where you left off.
🏥 Shared care planning note
If multiple organizations are involved in a shared Action Plan:
Only the organization that originally created the plan can archive it
If you’re not the originating org, reach out to the lead org to request archiving
We’re actively working on a “handover” feature that will allow another org to take ownership and archive the plan when needed
💡 Tips and things to know
Archiving does not delete any data—it simply hides the plan from the client’s view
Notifications and reminders will stop once a plan is archived
Archived plans are visible to healthcare providers only
