🩺 Why this matters
The Action Plan dashboard is the foundation of how Careteam supports person-centred, integrated care. It turns proven care pathways—or new ones—into actionable, adaptable templates, enabling both consistency and customization across teams and client needs.
Each Action Plan dashboard:
Reflects the workflow and focus of a care pathway (e.g., Hospital to Home, Palliative, Recovery)
Combines standardized building blocks with configurable design
Can be tailored by the health team to support individual client journeys
🔍 Where to find it
Go to the People tab
Select a person’s name to open their Action Plan dashboard
You'll see a personalized layout of blocks, reflecting the program or pathway they’re part of

👤 Person Panel (always at the top/left)
Includes:
Team members
Goals
Personal information (e.g., health card #, DOB)
Details about the Action Plan
This panel provides the full care context for the person at a glance.
📦 Blocks: The building units of every Action Plan
Each Action Plan is built using blocks you can customize based on team needs. These blocks can appear:
In the main panel (larger view)
Or in the side panel (compact view)
Common blocks include:
✅ Things to Do block– Central task/activity coordination
📄 Resources block– Documents, videos and educational content
📝 Check-ins block– Bringing the results of check-in to the main dashboard
📋 Checklist block– Key tracking
💬 General Information block– Shared updates for everyone involved
🔒 Status Updates block– Health team-only updates
📅 Important Dates block– Key moments in care (admission/discharge)
🔗 Services and Referrals block – Direct access to referral workflows
🏁 Discharge block - Supporting current status, discharge goals and current plan
Each dashboard is purpose-built to match a care program’s needs—for example:
Palliative care dashboards may highlight Check-ins and Status Updates
Hospital to Home may include Discharge Blocks, General Info, and Resources
Recovery Support may surface Referrals, Goals, and Client-led tasks

💡 Tips and things to know
Action Plan templates are created by your organization or program lead
Each plan can still be customized further for the individual client
All changes and updates happen in real time, so teams always have the latest info
As new Careteam blocks are added, you can enhance dashboards without disruption
