šŸ” What it is
Starting a new Action Plan in Careteam lets you onboard a new Person (client/patient) into your program or clinical pathway. With templates ready to go, setup is fast and simple—so you can spend less time on admin and more time on care.

šŸš€ How to start a new Action Plan

  1. Click ā€œ+ Start a new Action Planā€
    From the People we serve list, click the button to begin.

  2. Choose a template
    Select the appropriate Action Plan template from your organization’s list.

  3. Enter demographic details
    Fill in the Person’s information. Some fields are mandatory, such as name and date of birth.

  4. Customize if needed
    You’ll enter the Action Plan creator where most content is already pre-filled from the template.

  5. (Optional) Add invitations

    • Invite the Person by entering their email

    • Invite a primary supporter (e.g., family member or caregiver)

    • Confirm consent before sending invitations

  6. Activate or save as draft

    • Activate to go live and send invites

    • Save as draft if you're not ready to share yet

You can further tailor the Action Plan:

  • Add or edit tasks, resources, and forms

  • Update general information

  • Customize based on the Person’s goals or context

  • Templates save time by preloading standard content

  • You can use Careteam without inviting the Person or supporters

  • Adjustments are easy after activation—personalize as needed
