🔍 What it is
Archived Action Plans don’t disappear—they’re simply paused. When a Person completes a program or is temporarily discharged, their Action Plan can be archived. If they re-engage or return to care, you can quickly reactivate it.
🚀 How it works
To reactivate an Action Plan:
Go to the People tab
Click Archived to view people with archived Action Plans.
Find the Person
Select the Person whose Action Plan you want to reactivate.
Open the archived Action Plan
You can review the content without reactivating right away.
Click “Unarchive”
Within the About this Action Plan section, click Unarchive and confirm.
Once reactivated, the Person regains access, and the Action Plan appears in your active People list.

Retains all previous care history and updates
💡 Things to know
Only the organization that created the Action Plan can unarchive it
Reactivation sends a new invitation to the Person (if they had access before)
Perfect for re-engaging discharged patients or picking up where a care journey left off
