The Things to do block is one of the core elements of a Person’s Action Plan. It helps the Person and their Support Team know what to do next, and helps everyone involved in their care know what needs to be done and who is doing what.

Using the Things to do block:
To add Appointments, Check-ins, Forms, Tasks, or Referrals:
- Click the +Add button and choose what you’d like to add to the Things to do block as part of the Person’s Action Plan.
- Follow the guided steps to add the activity into the Person’s Action Plan
Key capabilities:
As a member of the Health Team, you can use the Things to do block to set up, update and monitor 5 different core actions that are needed to ensure the right things happen:
- Appointment: Schedule appointments with you or another provider or service. The appointment can be virtual with a secure access link, or in-person with directions to the location. The best part? The Person receives reminders the day before and an hour before the appointment, reducing no-shows and keeping them up to date with any changes or instructions on how to prepare.
- Check-in: Set up Check-ins to monitor how the Person is doing between visits. You can schedule Check-ins as one-time events or set them to recur regularly, allowing you to stay updated asynchronously and connect with the Person even when you’re not in the same room. Completed Check-in results are visible to the people of the Action Plan under the completed tab of the Things to do block.
- Form: Assign any Form (e.g., admission, follow-up) for the Person and their Support Team to easily complete online, on their phone, tablet or computer. We have a library of hundreds of standard forms to choose from such as PHQ-9, GAD-7 and many more. This lets the Person fill out a Form before seeing you, giving you more time for face-to-face interaction and inspiring confidence in their care. Form results are visible to the people of the Action Plan under the completed tab of the Things to do block.
- Task: Outline key activities that the Person and their Support Team need to complete to ensure they stay on track. You can specify what needs to be done and even indicate by when, so everyone stays on the same page and can coordinate efforts.
- Referral: If your organization already uses Caredove, you’re in luck—we seamlessly integrate with Caredove. Otherwise let us know and we can help connect you. It’s great because you can make referrals directly in Careteam through the Caredove integration. This ensures that everyone on the health and Support Teams knows what referrals have been made and their status, keeping everyone informed and engaged. - When you add an activity to an Action Plan—whether it's an Appointment, Check-in, Form, Task, or Referral —the Person and their Support Team are automatically notified. This ensures that everyone stays informed and up to date on the next steps in the care journey.
- Monitor activities in the Things to do block to see what’s coming up, what’s overdue, and what’s been completed. This ensures that everyone remains informed and on track.
