Action Plan templates are a powerful and essential feature within Careteam, designed to enhance standardization, operational efficiency, and the overall quality of care provided by healthcare providers. These templates serve as the foundation for creating personalized care plans around various health conditions, care pathways, or health promotion topics. In this knowledge base article, we will guide you through the process of creating Action Plan templates, empowering you to streamline your care delivery and ensure that each patient receives tailored and effective care.
- Click on the “Admin” tab.
- Click on the “Action Plan templates” link on the left side.
- Action Plan templates currently added to the library will be visible in the list.
- Click “Create template”.
- Create an Action Plan template name that will be known internally and click “Next”.
- Create an Action Plan template name that the patient and their supporters will see and click “Next”.
- Add any appointments and tasks to the template. Click “+ Add activity” and choose from the library list of tasks or appointments. Edit or fill in the details. Click “Add activity”.

Note: You can add a new task or appointment but it will not be saved in the library.

Add a check-in to the template. Click “+ Add Check-in” and choose from the library list. Determine who can see the check-in. Health Team only or everyone. Determine if the check in can be completed at any time, is to be scheduled or is recurring. Click “Submit check-in”. Add additional check-ins as needed. Click “Next”.

Add a form to the template. Click “+ Add Form” and choose from the library list. Edit the name the patient will see if you prefer. Determine if the check in can be completed at any time, is to be scheduled or is recurring. Click “Add Form”. Add additional forms as needed. Click “Next”.

Add resources to the template. Click “Add resource” and choose from the library list. Check the resources that you want to include in the Action Plan template. You can Click “Next”.

Add documents to the template. Select the documents from the library list that you want to include in the Action Plan template. To add a document click “Upload documents”. Locate the document on your computer, name the document and click “Save”. Click “Next”.

Write any notes and instructions for the patient and their support team as part of their Action Plan. The health care provider inviting the patient still has the opportunity to edit these instructions to personalize for the patient or remove unneeded content. You can securely communicate with members of your healthcare and support team on this platform whenever you need (1:1 messaging or group messaging) so everyone is on the same page. Click “Activate” if the template is ready to be used by the health team. Click “Save draft” if you still have work to do.
Edit or remove a Action Plan templates by following these steps:
- Click on the Action Plan template that you want to edit or remove.
- Make necessary edits. Click “Save” at the bottom.
- You can also choose to archive the Action Plan template which will remove it from the list viable to the health team or remove it all together.
