If you’re supporting patients through integrated palliative care, this is your guide.


🎯 Purpose of using Careteam to support the IPCP

Careteam makes it easy to securely share updates, resources, and team info—so everyone stays aligned on the plan and what the patient needs.

✔️ Enable integrated, person-centred care
✔️ Reduce back-and-forth communication
✔️ Create one shared care story and improve outcomes
✔️ Support families with clear next steps and shared visibility


🚀 Quick start guide

If you're a Team Assistant, starting a plan:

  1. Open a new Action Plan and enter accurate patient demographics.

  2. Add the appropriate Integrated Palliative Care Coordinator.

  3. Invite the appropriate partner organizations to the plan.

If you're joining a plan:

1️⃣ Accept the Invite — You'll receive an email when invited to a shared Action Plan. Click to accept and join.

2️⃣ Review the Action Plan Dashboard — Get up to speed quickly by checking key updates, completed and upcoming tasks, and who's on the team. (See the Dashboard Orientation video above.)

3️⃣ Add Your Support — Make meaningful contributions by sharing updates with the team, patient, and family; updating the Service Plan and Equipment Checklist; flagging questions using Status Updates, PPS, and ESAS; and adding Tasks and Resources.

🔑 For Intake Receivers

If your organization receives Action Plan invitations, the Intake Receiver reviews and accepts or declines them on behalf of your team. (See the Intake Receiver video above.)

It helps to know what comes next in your care. We use a secure digital platform called Careteam. It keeps your care plan in one place. Today, we will send you an email. Please open the email and accept the invite. Your Action Plan will show you the next steps for Mississauga Health Integrated Palliative Care Program. We will update it when things change. You may get a message when there is an update. Please check Careteam often. It is the best place to see your care plan. You can also invite people who help you, like family or care providers. Do you have any questions you would like to ask about using Careteam?
