Unlock the full potential of collaboration in Careteam by inviting support and health team members to join a person's Action Plan. For health and support team contacts who have been added discover how to extend an invitation, granting them access to Careteam for more effective and streamlined collaboration.
- Click on the “People” tab to see the list.
- Select the “Person's” name.
- Select the appropriate Action Plan if there are more than one.
- Click on the “Team” tab.
- Identify the contact and click on “+ Invite”.
- For support team members select if the contact is to be a primary supporter or a supporter.
Primary Supporter (most access): A trusted family member or friend. They can communicate with the health care team, view health information, send messages, complete tasks, and upload documents. Supporter: Usually relatives or friends. They can view health information, send messages, complete tasks, and upload documents. - Confirm the contact's email and that you have received consent from the patient to allow this person to access the patient's personal health information and act on behalf of them through Careteam.
- Click “Send invitation”.
