A quick guide to identifying and updating the lead provider for a patient’s/clients Action Plan

🩺 Why this matters

Clearly identifying the Most Responsible Provider (MRP) ensures:

  • Clear clinical leadership

  • Safer, more coordinated care

  • Better communication across providers

  • Accountability when care is shared or transferred

Since roles can shift frequently, Careteam makes it easy to update the MRP for each Action Plan—whether temporarily or permanently.


🔍 Where to find it

  • Go to the People tab

  • Select the patient and open the relevant Action Plan

  • Click the Team page

  • Look under the Health Team section


🚀 What you can do in this section

🎯 Assign the Most Responsible Provider

  1. Click on the Health Team member you want to assign as MRP

  2. Click Assign as Most Responsible Provider

  3. Click Confirm

🔁 Reassign the MRP

  1. Click on the current MRP (indicated by a blue star beside their name)

  2. Click Un-assign and then Confirm

  3. Click on the new provider

  4. Click Assign as Most Responsible Provider and Confirm

🌐 Assign an MRP across organizations
You can also assign a provider from another organization as the MRP.

  • They’ll be notified and asked to confirm

  • You’ll see a status like “Not yet confirmed by [provider]” so you’re always informed


💡 Tips and things to know

  • The MRP can change per Action Plan

  • The assignment will appear to all team members in that plan

  • If you assign someone else as MRP, they must confirm the role

  • Use this feature to support clear handoffs and transitions between providers

The MRP feature ensures everyone knows who’s in the lead—so care stays safe, coordinated, and responsive.
