🌐 Assign tasks & appointments across organizations

You can now assign tasks and appointments to providers outside your organization — making cross-team collaboration smoother and supporting more integrated models of care.

👥 Population Management: Bulk updates to Action Plans

With Population Management, you can now update multiple Action Plans at once using group actions.

This includes contributing to:

• Status Updates

You can apply changes across multiple individuals simultaneously — including updates to the Health Team or the Person and their support team — helping streamline large-scale care coordination.

🔧 Enhancements

Improvements to existing features to make workflows smoother and clearer.

🔔 Upgraded Careteam notification centre

A refreshed in-app notification experience:

• Mark items as read or unread

• Clear notifications from your active list

• View previously cleared items anytime

This gives you more control over your notification feed and helps reduce clutter.

🔐 CAPTCHA implemented across the app

We’ve introduced CAPTCHA across the platform to enhance security and protect against automated or unauthorized access — helping keep Careteam safe and reliable for all users.

💬 Secure messaging: Improved channel naming

In Secure Messaging, channel names now default to displaying:

• The person’s name

• Their date of birth (D.O.B.)

This makes it easier to quickly identify the correct conversation.

🧭 Updated “Discharge” block → “Coordination” block

Based on user feedback, we’ve updated the Discharge block to the Coordination block.

While the functionality remains the same, the updated terminology better reflects ongoing care workflows. The block now includes:
