🩺 Why this matters
Setting clear, meaningful goals is central to person-centred, integrated care. Careteam makes it easy for both the health team and the Person (or their primary supporter) to create, track, and update goals—so everyone can focus on what matters most.
Use goals to:
Encourage motivation and accountability
Foster collaboration between patients, families, and providers
Personalize care beyond clinical outcomes
🔍 Where to find it
Go to the People tab
Select the Person’s name
Open the relevant Action Plan
Look for the “Person’s Goals” section in the Person Panel
🚀 How to add a new goal
Click + Add Goal
Enter a title (e.g., “Take a flight,” “Walk without pain,” “Attend graduation”)
(Optional) Add a description to provide context
(Optional) Upload a photo for motivation
Click Save
✅ The goal is now live in the Action Plan and visible to the entire team.
🛠️ How to update or complete a goal
Click on the goal to edit or update its details
When a goal is achieved, click “Mark as Achieved”
Use the toggle to hide/show completed goals for a cleaner view

You (as a healthcare provider)
The Person themselves
Their Primary Supporter
Encouraging clients or families to set their own goals can boost motivation and build ownership in the care process.
💡 Tips and things to know
Visual cues like photos or milestone titles make goals more personal and memorable
Goals live in the Person Panel, so they’re always visible and top-of-mind
Great for any program—rehabilitation, recovery, chronic care, palliative, and more
Goals are not tied to clinical forms—they reflect what matters to the person
