Action Plans

🧭 What does a Person’s Action Plan look like?

Explore how clients, families, and health teams interact with the same Careteam dashboard—with role-specific permissions.

Updated 1 week ago

When you activate an Action Plan for a person, everyone—Health Team, Support Team, and the Person—shares a consistent layout. That’s intentional. A shared interface promotes clarity, reduces friction, and helps everyone stay on the same page.

💡 Why This Matters

  • Clarity for everyone: All care participants—patients, caregivers, providers—see the same layout.

  • Role-based permissions: People only see or edit what’s appropriate for their role.

  • Confidence and trust: Shared visibility reduces duplicate work, improves follow-through, and builds relationships.
