(Client name or caregiver), we know that navigating (use case, e.g. a health journey, recovery, end-of-life care) can feel overwhelming at times—for you and those supporting you. Clear communication and shared information make a huge difference. In fact, research shows that when care teams and families have a shared plan and access to updates, people feel more supported and in control. A shared plan is a place where you can see everything that matters—appointments, tasks, check-ins, helpful resources—so everyone knows what’s happening, what’s coming up, and how to help.

That’s why we use a secure app called Careteam. It’s a web-based application that works on your phone, computer, or tablet. It acts as a central hub for your care—where you can see your Action Plan, keep track of next steps, and access important resources. Think of your Action Plan as an action-oriented care plan. It helps you stay organized and makes it easier for others to support you.

We encourage you to use Careteam as your go-to place—your digital binder throughout this journey. You’ll get email or text notifications when new updates are added, and it’s a good idea to check in regularly to stay up to date. It’s also a place where you, your care team, and any support people you choose can stay connected.

We’ll be sending you an email invite to join Careteam today. Please accept it—it’s an important part of how we’ll keep things clear and coordinated. Your Action Plan will serve as your guide through (use case), and we’ll keep updating it as your needs evolve. Once you’re in, you can also invite any family members or support people you’d like to involve in your care.
