A shared plan is a place where you can see everything that mattersāappointments, tasks, check-ins, helpful resourcesāso everyone knows whatās happening, whatās coming up, and how to help.
Thatās why we use a secure app called Careteam. Itās a web-based application that works on your phone, computer, or tablet. It acts as a central hub for your careāwhere you can see your Action Plan, keep track of next steps, and access important resources. Think of your Action Plan as an action-oriented care plan. It helps you stay organized and makes it easier for others to support you.
We encourage you to use Careteam as your go-to placeāyour digital binder throughout this journey. Youāll get email or text notifications when new updates are added, and itās a good idea to check in regularly to stay up to date. Itās also a place where you, your care team, and any support people you choose can stay connected.
