Phase 2 of Careteam roll-out in PIVOT: Improve cross-team communication by enabling collaboration with community partners
The PIVOT Dashboard brings together key capabilities to streamline communication and coordination for pediatric IV outpatient therapy. Each capability is designed to improve efficiency, reduce duplication, and enhance collaboration between hospital teams and community partners.
General Information – A centralized space where the health team can add important details to share with other team members and the patient/family. While patients and families can view this information, only the health team can make updates, ensuring clarity and consistency.

Things to Do – A section for adding key activities that need to be completed by the patient/family or the health team. Tasks and appointments are the main activities, helping to ensure timely follow-ups and coordination of care.

Check-Ins Results– Patients and families receive a daily check-in to provide updates on the dressing, central line, and fever. The health team will receive an in-app notification when a check-in is completed and can review the results in real time.

Status Updates – A space for the health team to communicate important updates regarding the patient’s care. These updates are only visible to health team members, ensuring secure and professional coordination.

Create an Action Plan:
Click "+ Start New Action Plan."
Enter the patient’s name, date of birth, personal health number (PHN), and proceed through to invitations.
Add the patient’s primary supporter’s email as the primary user of Careteam.
Send the invite
Adjust the Action Plan:
Modify activities and resources as needed to align with the patient’s care plan.
Script for Introducing Careteam to Patients and Families:
"(Patient name or primary caregiver), it's really important that everyone helping with your care stays connected as you go home. We use a safe tool called Careteam that makes it easy for you and your health team to share information and work together. We’d love to include you! Can I send you an email invite?"
"Great! You'll get an email invite soon. Just follow the steps to join. Once you're in the PIVOT Dashboard, you'll see instructions on what to do next, plus helpful resources, documents, and other important information from today. You can also invite family or supporters to be part of your care team!"
