Is Careteam an Electronic Medical Record?

Careteam enables coordination and collaboration within and between health organizations, including the patient.

Careteam is not an Electronic Medical Records (EMRs). EMRs are “systems of record”, mostly documenting care that has been provided within one organization.

Careteam does not replace an EMR system, rather the two are complementary. Careteam connects to modern Electronic Medical Records (EMRs), it is not just a way to transfer information between EMRs or put all the EMR information into one place.  Rather, it is focused on activating the proactive plan of care across a distributed team. Careteam is a “system of action”, empowering your client and their personal support team, and bringing the whole health and social care team together around the person with a shared care plan and communications across organizations. The EMR is more of an internal system of record, where Careteam is used to facilitate collaboration with the client and their family/caregivers and coordination with other organizations.
