This guide is for BANAC staff and partners working together to support shared clients through the Lower Limb Preservation Pathway.


🎯 Purpose of using Careteam to support the Lower Limb Preservation Pathway

Careteam gives BANAC one shared, secure workspace to coordinate lower limb preservation care around the client — instead of the phone tag, faxes, and repeated updates that come from each organization communicating separately.

✔️ Coordinate care live, across organizations, on one dynamic shared care plan ✔️ Give the client a direct role in their own care, including daily self-reporting ✔️ Replace phone tag and faxes with a single, up-to-date source of truth ✔️ Keep partner organizations and family members aligned as care progresses


🧭 What Careteam is (and is not)

Careteam is:

  • A shared, live workspace for cross-organization collaboration

  • A place to share updates, tasks, resources, and goals of care

  • A space where the client can report on their own care, like a daily foot check

Careteam is not:

  • ❌ An EMR

  • ❌ A replacement for your clinical documentation and regulatory record-keeping

BANAC continues to document clinical encounters in its own EMR. Careteam replaces what typically happens outside the EMR — the spray-and-pray updates, telephone tag, and repeated conversations that come with coordinating across organizations and with the client.


🚀 Quick start guide

1️⃣ Log in

Go to careteam.me/login and choose your organization (BANAC). You'll land directly on the list of clients your organization supports. Save the login page as a bookmark or a home screen icon so it's easy to get back to.

2️⃣ Start a new Action Plan

Go to Start New Action Plan, choose the Lower Limb Preservation Pathway, and click Create. Date of birth and name are required — the more information you add, the better. Click Next.

Scroll all the way to the bottom of the setup screen and click Finish and Activate. This step is easy to miss, and skipping it will cost you time later.

3️⃣ Build the team and invite the client

Go to Team, click Add to Team, and add a support team member's first name, last name, relationship, and email — with the client's consent. Invite partner organizations the same way as they join the client's care. Then go to Practice's Information to send the client their own invite, so they can start reporting and collaborating directly.


🔑 How Action Plans are created and shared

  • BANAC staff start Action Plans for clients in the Lower Limb Preservation Pathway

  • Practice Pete is your practice client — his sample Foot and Wellness Action Plan shows what a live plan looks like before you work with a real client

  • Once activated, invite the client, add family members with consent, and invite partner organizations (like Rama Healthcare) as they join the client's care

  • If a client or partner hasn't accepted their invite yet, resend it from Practice's Information

  • When a client is discharged, go to About This Action Plan and click Archive This Action Plan — no need to leave it open


🤝 How collaboration works across the team

Careteam replaces fragmented communication with one live Action Plan. When one organization posts an update, everyone invited sees it right away.

Team Shows everyone on the client's care team, including their organization. Add family members with the client's consent, and invite partner organizations as they come on board.

General Information The shared bulletin board — geared to the client and their support team. A welcome message is pre-populated, and you can add your own updates, like a reminder about a daily foot check. The client gets notified whenever it's updated.

Status Updates (health team only) Key updates for the health team only — not the SOAP note, just what the team needs to know, like a wound to keep an eye on. Hidden from the client and their support team. Shows the latest three updates, with a View All option for more.

Care Coordination Where the team collaborates on current status, next steps, and goals of care. Click into the history to see who updated what and when.

Things To Do Turns next steps into action — appointments and tasks assigned to anyone on the team, even across organizations. Tag or assign a specific person so they're notified directly, and mark items complete as they're done.

Resources A starter set of key resources shared with the client, plus a library you can pull additional tools from — like a foot screener. Add one-time resources too: documents, videos, or websites.

Daily Foot Check Unique to this pathway — the client completes a daily foot check, and the dashboard shows their responses as a trend over time, with free-text notes for anything they've noticed (like a wound changing in size).

Messaging Secure messaging built around the Action Plan. Start a channel with anyone on the client's team, across organizations.


👤 About client and family involvement

Clients and family members are invited directly into the Lower Limb Preservation Pathway:

  • Clients can add and update their own goals of care

  • Clients complete the Daily Foot Check themselves — self-reported data the whole team can see

  • Family members can be added to the team with the client's consent

  • Status Updates stay hidden from the client and family — that block is health team only

📌 Pro tips for success

  • Don't forget to click Finish and Activate at the bottom of the setup screen when starting a new Action Plan — it's the step people miss most

  • Check the Daily Foot Check trend regularly — it's the fastest way to catch a change before it becomes a bigger problem

  • Try the workflow with Practice Pete before working with a real client

  • Archive an Action Plan as soon as a client is discharged, from About This Action Plan
